Early Alerts If you would like to refer a student for Early Alerts please fill out the referral form below.Student Referral Form Purpose of the Committee / How to Refer If you are concerned about anyone who is an undergraduate student at Hamline, please fill out our referral form. You may refer someone to the Early Alert Committee for a variety of reasons. You may know of a friend who is consistently skipping meals, and you're concerned about his health. Perhaps you know of someone who may be developing a substance abuse problem. Maybe you know a first-year who is having too much fun and is skipping too many classes. If you're concerned, feel free to use the student referral form. Confidentiality All submissions will be kept confidential. Based on the information you provide, a member of the Early Alert team will follow up with the student. Because of the confidential nature of the referral, the Early Alert Committee is unable to send updates on referrals made using the online submission form.If you'd like more information, please contact Patti Klein, the Assistant Dean of Students, at 651-523-2421. Committee Description The Early Alert Committee is comprised of staff members from offices across the university. The group meets weekly to touch base regarding undergraduate students who may benefit from targeted outreach from a particular office. Represented offices include: Admission Office Athletics Center for Academic Services Center for Student Success and Transition CLA Deans Office Counseling & Health Services (ex-officio member, receive information only) Dean of Students Office Disability Services (ex-officio member, receive information only) Financial Aid Hedgeman Center International & Off-Campus Programs Religious and Spiritual Life Office in the Wesley Center Residential Life Student Accounts Student Leadership and Activities. The group is confidential. Any information regarding a student is kept confidential. At no time does a referral to the Early Alerts Committee affect a student’s official transcript or standing with any part of the university.
